Eligibility requirements at a glance
Michigan resident living in a home or apartment, not a nursing facility
Enrolled in Medicaid before services can be authorized
Need for hands on help with daily activities
A required face to face assessment with an adult services worker
Activities reviewed for eligibility
Activities of Daily Living: bathing, dressing, toileting, mobility, eating, grooming
Instrumental Activities of Daily Living: meal preparation, light housekeeping, laundry, shopping for essentials when appropriate
Required forms
Adult Services Application (DHS 390). Completed and signed by the applicant or their legal representative. This form starts the case with the state.
Medical Needs Form (DHS 54A). Completed and signed by a licensed health care professional. This form confirms that help with daily activities is related to a medical condition. It must be current, signed, and dated.
How eligibility is determined
Intake collects basic information about needs and living situation

A face to face assessment documents the help needed with daily activities
A plan of service lists approved tasks and the number of hours for each task
Reviews occur on a regular schedule and the plan can be updated if needs change
