How to Update Your Information with MDHHS for the Home Help Program
If you are part of the Michigan Home Help Program, keeping your information current with the Michigan Department of Health and Human Services (MDHHS) is important. Even small changes, like a new address or phone number, can affect your eligibility or cause payment delays.
Updating your information ensures MDHHS can contact you about your care, renewal, or payment status.
Care Plan Inc. helps Michigan caregivers stay organized and report updates correctly so they continue to receive weekly pay at $17 or more per hour once hours are approved.
Why It’s Important to Update Information
MDHHS uses the information in your case file to confirm your eligibility, approve care hours, and send notices about renewals or reassessments. When your information is outdated, important mail may not reach you, or your file may not match your current situation.
For example, if you move and forget to update your address, your renewal notice could go to your old home and cause your case to close.
Keeping your details current helps avoid interruptions in services and payments.
When You Should Update Your Information
You should contact MDHHS right away when any major change occurs in your life or in the care recipient’s situation. Updates are required whenever the following happen:
Change in address or phone number
New caregiver or change in caregiver relationship
Change in household members
Medical condition improvement or decline
Medicaid renewal or coverage changes
Hospitalization or long-term facility admission
Income or benefit updates
If you’re unsure whether something needs to be reported, it’s safer to notify your Adult Services Worker, often called an ASW.

How to Report Updates to MDHHS
There are several ways to report updates, depending on the type of change. The fastest option is usually to contact your local MDHHS office directly or use your MI Bridges account online.
Here’s how to do it:
By Phone: Call your local MDHHS office and ask to speak with your ASW or caseworker. Provide your name, case number, and updated details.
By Mail: Write a short letter including your case number, the update, and your signature. Send it to your assigned MDHHS office.
Online: Log into your MI Bridges account to update contact information or check case status.
In Person: Visit your MDHHS office if the change involves forms or identification.
Keep a copy of anything you submit, including the date and method you used to report it.
How Updates Affect Caregiver Pay
When information is not updated, payments may be delayed or paused. For example, if a caregiver changes but the new caregiver is not enrolled in CHAMPS yet, MDHHS cannot issue payments until that process is complete.

Similarly, if your medical condition changes, MDHHS may need a new DHS-54A Medical Needs Form from your doctor before continuing payments.

Keeping Your Medicaid Active
Many Home Help participants experience interruptions in service when Medicaid renewals are missed. Always check your mail for renewal forms and respond as soon as possible.
If your Medicaid lapses, your Home Help services and caregiver payments will pause until coverage is reinstated.

Example Situations
Here are a few examples of when to report changes right away:
You move to a new home and need your ASW to update the care address.
Your doctor diagnoses a new condition that affects your care needs.
Your caregiver changes, and the new caregiver must register in CHAMPS.
You renew Medicaid but have a new ID number or case reference.
Reporting these updates quickly keeps your case active and your caregiver payments consistent.
What Happens After You Report a Change
After MDHHS receives your update, your ASW will review it and decide if a new assessment or updated documentation is needed.
For example, if you report a change in health, your ASW may request a new medical form or schedule a short visit to reassess care needs.
If everything checks out, your case will continue without interruption.
